Provider Participation Standards

Senior care providers who would like to become a member of the CareCrunch network and participate in its referral program must meet the follow eligibility requirements:

• Complete and submit the CareCrunch provider application
• Provide evidence of proper licensure or certification
• Provide evidence of General Liability and Medical Malpractice Insurance, if applicable
• Provide names and contact information of reference if required by CareCrunch
• Accept the terms within the CareCrunch Provider Agreement

CareCrunch reserves the right to deny participation for any provider who 1. fails to provide CareCrunch with satisfactory evidence of any of the required information above, or 2. is discovered through references or background checks to have a record that would place CareCrunch at risk by referring consumers to said provider.
